Please complete the required form(s) with the bibliographic information for the items you are requesting to place on physical or electronic course reserves. Incomplete forms will delay processing of your request. A Fair Use Checklist is employed to assist you in determining whether access to the item is governed by the Fair Use Doctrine, as adopted by the University Systems of Georgia, and must accompany EACH item requested for course reserves. It is the professor's responsibility to determine Fair Use for each item and/or obtain copyright permissions as necessary.
The E-Reserve form can be found and filled out here: E-Reserve Form (libwizard.com). Please complete the form at least three (3) business days prior to assigning readings to students.
