FACULTY USE ONLY
Please complete the required form(s) with the bibliographic information for the items you are requesting to place on physical or electronic course reserves. Incomplete forms will delay processing of your request. A Fair Use Checklist is employed to assist you in determining whether access to the item is governed by the Fair Use Doctrine, as adopted by the University Systems of Georgia, and must accompany EACH item requested for E-Reserves.
